Our services are provided at our facility located at 407 C1, Nomis Weragala Mw, Hokandara South, Thalawathugoda, Sri Lanka. We reserve the right to modify, suspend, or discontinue any service at any time with prior notice to residents and their families.
3. Admission and Enrollment
3.1 Admission Requirements
To be admitted to our elder care facility, the following requirements must be met:
- Complete application form with accurate information
- Medical assessment and health records
- Selection and payment of appropriate care package
- Emergency contact information and legal guardian details
- Signed service agreement and consent forms
3.2 Facility Tour and Assessment
We encourage prospective residents and their families to visit our facility before admission. During the visit, our team will conduct a comprehensive assessment to determine the appropriate level of care and ensure our services meet the resident's needs.
4. Payment Terms and Fees

Payment for services is due according to the schedule agreed upon in the service contract. We accept payments via our secure online platform, bank transfer, or other approved methods. Late payments may incur additional fees as specified in your service agreement.
4.3 Deposits and Refunds
A security deposit may be required upon admission. Deposit refund policies, including conditions and timelines, will be detailed in your service agreement. Refunds are subject to the terms outlined in the contract and may be adjusted for any outstanding balances or damages.
5. Resident Rights and Responsibilities
5.1 Resident Rights
All residents have the right to:
- Dignified, respectful, and compassionate treatment
- Privacy and confidentiality of personal information
- Participate in care planning and decision-making
- Access to medical records and care documentation
- Voice concerns and grievances without fear of retaliation
- Regular family visits and communication
5.2 Resident Responsibilities
Residents and their families are expected to:
- Provide accurate and complete health information
- Respect staff members, other residents, and facility property
- Follow facility rules and care plans
- Make timely payments as agreed
- Notify us promptly of any changes in health status or needs
6. Medical Care and Health Services
6.1 Healthcare Coordination
We coordinate with healthcare providers to ensure comprehensive medical care for our residents. This includes regular health assessments, medication management, and coordination with external medical specialists when necessary.
6.2 Medical Emergencies
In case of medical emergencies, our staff will take immediate action to ensure the resident's safety and well-being, including contacting emergency services and notifying family members or designated contacts. We are not responsible for decisions made by emergency medical personnel or external healthcare providers.
6.3 Medication Management
Our trained staff manages medications according to physician orders and best practices. Families are responsible for ensuring timely supply of prescribed medications and must inform us of any changes in prescriptions.
7. Digital Platform and Online Services
7.1 Account Access
Upon enrollment and payment, family members will receive access to our digital platform where they can view updated medical records, care plans, and other relevant information about their loved one. Account credentials are personal and should not be shared.
7.2 Website Usage
You agree to use our website and digital platform only for lawful purposes. You must not use our services to:
- Transmit harmful or malicious code
- Attempt unauthorized access to our systems
- Violate any applicable laws or regulations
- Share confidential information of other residents
8. Termination and Discharge
8.1 Voluntary Discharge
Residents or their legal representatives may terminate services with written notice as specified in the service agreement. Deposits and refunds will be processed according to the terms outlined in the contract.
8.2 Involuntary Discharge
We reserve the right to discharge a resident under the following circumstances:
- Non-payment of fees after reasonable notice
- Resident's needs exceed our care capabilities
- Violation of facility policies or endangering safety
- Facility closure or service discontinuation
In all cases, we will provide reasonable notice and work with families to ensure appropriate transition arrangements.
9. Liability and Insurance
9.1 Limitation of Liability
While we maintain high standards of care and safety, Care365 is not liable for:
- Loss or damage to personal belongings (residents are encouraged to maintain personal property insurance)
- Medical outcomes resulting from pre-existing conditions or natural disease progression
- Injuries sustained despite reasonable precautions
- Actions of third-party healthcare providers
9.2 Insurance Coverage
Care365 maintains appropriate liability insurance as required by Sri Lankan regulations. We recommend that residents and families maintain their own health insurance and personal property insurance for comprehensive protection.
10. Visitor Policy
We welcome family members and friends to visit our residents. However, to ensure the safety and well-being of all residents, visitors must:
- Adhere to visiting hours as posted
- Sign in at reception upon arrival
- Respect privacy of other residents
- Follow health and safety protocols
- Not visit if feeling unwell or showing symptoms of illness
11. Intellectual Property
All content on the care36t5.com website, including text, graphics, logos, images, and software, is the property of Care365 or its content suppliers and is protected by Sri Lankan and international copyright laws. You may not reproduce, distribute, or create derivative works without our express written permission.
12. Compliance with Laws
Our services comply with all applicable Sri Lankan laws and regulations, including those governed by the Sri Lanka Private Health Services Regulatory Council. We maintain necessary licenses and certifications to operate as an elder care facility.
13. Dispute Resolution
In the event of any dispute arising from these Terms and Conditions or our services, we encourage open communication and mediation. If a dispute cannot be resolved through discussion, it shall be subject to the exclusive jurisdiction of the courts of Sri Lanka.
